This is a rare form of mental illness in which a person has more than one personality, each dominating at different times. The personalities are usually completely different. The image may depict schizophrenia, a state of deluded thinking in which reason and logic become distorted. Some schizophrenics also suffer from hallucinations. Alternatively the image could be showing transfiguration, when the spirit departs the body.
